Required Application Documents Checklist
Please have these items ready for all household members before applying.
Government Photo IDs Valid state ID or driver's license for all household adults age 18+.
Social Security Cards Official SSN cards for every member of the household.
Proof of Income Last 4 consecutive pay stubs, SSI/SSDI benefit letters, or TANF statements.
Birth Certificates Official birth certificates for all minor children under age 18.
